Title: Exploring the Impact of Endometriosis on Intimate and Sexual Relationships: A Qualitative Study

Abstract

Endometriosis is a chronic gynecological condition that significantly affects multiple domains of women’s health, including physical functioning, emotional well-being, and interpersonal relationships. While prior research has extensively documented the medical and pain-related aspects of the condition, less attention has been given to its impact on intimacy and sexual relationships from the patient’s perspective. This qualitative study explores the lived experiences of 29 Puerto Rican women diagnosed with endometriosis, focusing on how the condition shapes their intimate and sexual relationships. A reflexive thematic analysis approach (Braun & Clarke, 2006, 2019) was used to identify patterns of meaning across participants’ narratives. Findings revealed that dyspareunia was reported with high intensity in 88% of references, significantly affecting sexual functioning and desire. Additionally, 62% of references reflected negative couple experiences, including relational strain and reduced intimacy. Emotional dimensions were also prominent, with 48% of references indicating challenges in empathy and emotional connection within the relationship. Participants expressed unmet educational needs (54%), particularly regarding communication, symptom understanding, and relational coping strategies. Furthermore, 83% of references highlighted negative self-esteem, often associated with altered body image and perceived sexual inadequacy, while 72% described negative fertility-related experiences. These findings underscore the multidimensional impact of endometriosis, extending beyond physical symptoms to deeply affect relational and psychological functioning. The study highlights the need for integrative care models that incorporate psychological support, couple-based interventions, and patient-centered education to improve overall quality of life.
